Please use this form to provide us with important medical information and permissions for your child.

Sevenoaks School is committed to providing opportunities to all children, whatever medical condition they may have. We can only do this with full disclosure and information provided by parents and medical professionals involved in the student’s care, so please answer the below questions as fully as possible.

Please do not send over the counter medication, supplements or herbal remedies with your child as we are unable to administer these. Any sent will be collected from your child, held securely and then returned with your child at the end of the course. As the current UK government guidance states that Covid testing should not be routinely carried out, please do not send your child with Covid tests.

Please use this form to provide us with important information about your child’s inbound and outbound arrangements.

Please note that we can only provide transfers from the airports and railway stations listed and that the service is for students only. As your child will be transferred to Sevenoaks with other Summer Programmes students we unfortunately cannot guarantee family members a transfer.

Please book your travel arrangements to correspond with the below times:

Session One:

3 July: please arrive between 9:00 and 16:00 UK time.

17 July: please depart between 07:00 and 12:00 UK time.

Session Two:

17 July: please arrive between 12:00 and 16:00 UK time.

31 July: please depart between 09:00 and 17:00 UK time.
